Job Title: Community Health Worker FT/Non Exempt (40 hours/week)

Salary: $22 per hour
Rolling Start, Inc. (RSI) Center for Independent Living is a 501(c)(3) non-profit organization that provides advocacy and services to people with any disability in San Bernardino, Inyo and Mono counties. The mission of RSI is to educate and empower people with disabilities to achieve the independent life of their choice. We have been serving consumers for 45 years.
Position Description:
Under the direction of the Program Manager, the duties, functions, and requirements of the Community Health Worker (CHW) are as follows: This individual will support all ADRC and ILC staff with public health efforts related to COVID-19 such as assistance with getting vaccinations and boosters, activities around mental health, assistance for people who have contracted COVID, and preparing consumers for public health emergencies. The CHW will work collaboratively with 211, food banks, food distributors, county behavioral health, county public health, health plans and other community organizations to promote healthy nutrition, disease management, public health safety and mental health. The CHW will participate in community outreach activities, provide education and training (virtual and in-person) while supporting public health education campaigns. This individual will assist consumers in navigating their healthcare by helping them successfully participate in their medical and/or behavioral health benefits, overcoming barriers to care, and sharing information on barriers with RSI leadership for quality improvement.
Essential Duties and Responsibilities:
Ability to maintain confidentiality;
Support public health efforts to educate the community on infectious diseases;
Delivering information about health and wellness in ways that the community can easily understand;
Establish positive and supportive relationships with consumers;
Motivate consumers to be active and engaged in their health;
Ability to objectively report data and identify targeted areas of program activity.
Qualifications and Skills:
High school diploma or GED required;
Must demonstrate knowledge of the Independent Living movement, and Philosophy;
Two (2) years of experience working in social work, community outreach, or customer service;
Must successfully complete an approved Community Health Worker program within 12 months of hire;
Knowledge of the needs and barriers people with disabilities and seniors face in their community;
Solid understanding of the available resources in San Bernardino, Inyo and Mono counties;
Ability to communicate clearly and at a professional level both verbally and in writing; a commitment to professional growth;
Bi-lingual in English and Spanish;
Must be fully vaccinated for Covid-19 and provide vaccination record.
Other Preferred Qualifications:
Knowledge of the services in the areas. Sensitivity to, and knowledge of, the needs of persons with disabilities;
Knowledge of Rehabilitation Act of 1973 and the Americans with Disabilities Act, Work Incentives Act, and IDEA preferred
Bilingual speaking skills a plus
Good math and writing skills
Good phone skills
Good computer skills
Work Conditions:
The physical demands will vary. The noise level of the office working condition is low to moderate. It is anticipated 60% of the time will be in the community which will be a variety of indoor and outdoor settings, e.g. consumer homes, community/senior centers, community organizations, health plans, farmers markets, resource fairs, etc. Travel throughout our service area is expected, statewide travel is possible for trainings. Must be able to sit for long periods of time, handle multiple tasks at one time, lift and carry up to 50lbs.
